Am I eligible for Restylane RewardsSM?

What is Restylane Rewards® Premium Status?
- After you have received five qualifying treatments as a Restylane RewardsSM member, you achieve Premium Status. As a Premium Status member, you earn a $200 treatment voucher towards a future Restylane® treatment from a participating provider after you use a cumulative 4.0 mL of product (click here for examples). Additionally, you earn a $25 Restylane Rewards® Benefit Check for every Restylane treatment using at least one 1.0 mL or two 0.4 mL syringes (minimum allowable time between office visits for a Restylane treatment is 30 days).
Can I receive a $25 Restylane RewardsSM Benefit Check from a treatment using 0.4 mL of Restylane® as a member with Premium Status?
- No, in order to receive a Restylane RewardSM Benefit Check for treatments received as a member with Premium Status, you must use at least one 1.0 mL or two 0.4 mL syringes (a minimum of 0.8 mL) of Restylane® during your treatment. You will receive credit toward the $200 treatment voucher for the 0.4 mL of product as long you submit the end flap from the Restylane box that contains the Medicis Aesthetics™ logo. What is a qualifying treatment?
- Any Restylane® treatment you receive as a registered member of the Restylane RewardsSM program is eligible for submission. Qualifying treatments are those that have been submitted and validated by the requested treatment documentation. To receive a reward, your treatment must use at least one 1.0 mL or two 0.4 mL syringes of Restylane.
How do I track my status for earning my voucher as a member with Premium Status?
- You can track the number and sizes of syringes used in validated Restylane® treatments by visiting the Restylane Rewards® section of www.RestylaneUSA.com and clicking on My Restylane Rewards, or by calling toll-free 800-211-6794, 9 a.m. to 5 p.m. Eastern time. You must submit a box end for each syringe of product used in your treatment to receive credit toward the $200 treatment voucher.
Do I need to submit an end flap for each syringe used in my treatment?
- Yes, you must include an end flap from each Restylane® box used in your treatment for your reward to be processed. You must submit a box end for each syringe of product used in your treatment to receive credit toward the $200 treatment voucher.
Why do I need to include the end flap(s) from the Restylane® box(es) with my submission?
- Providing the end flap(s) from the Restylane® box(es) that contain the Medicis Aesthetics™ logo with your submission is for your protection. This ensures you have received genuine FDA-approved Restylane.
Do I need to register with RestylaneUSA.com to be eligible for Restylane RewardsSM?
- Yes, you must be registered in the Restylane RewardsSM program on RestylaneUSA.com.
How do I register with RestylaneUSA.com?
What are the requirements to receive my reward?
- Register on RestylaneUSA.com and create a User Name and Password.
- Receive a Restylane® treatment (using at least one 1.0 mL or two 0.4 mL syringes) from your physician.
- Obtain a receipt from your provider documenting treatment with Restylane® (credit card receipts cannot be accepted as proof of treatment).
- Have the provider give you the end flap(s) from the Restylane® box(es) used in your treatments showing the Medicis Aesthetics logo (click here for examples).
- Complete the Restylane Rewards® Redemption Form.
- Mail the completed Restylane Rewards® Redemption Form, including User Name on RestylaneUSA.com, the provider's treatment receipt (copies are okay), and the end flap from the Restylane® box(es) to the Restylane Rewards Center.
- Postmark your submission within 30 days of treatment.
Can I choose a gift card for my reward as I may have done previously?
- The value of your earned reward for the first five qualifying treatments as a Restylane Rewards® member has not changed. However, to expedite the processing of rewards submissions, Restylane Rewards are now issued as Restylane Rewards Benefit Checks.
Has the submission process changed now that rewards are issued as Restylane Rewards® Benefit Checks?
- No, the requirements to receive and submit a reward remain the same.
Where can I find the Restylane Rewards® Redemption Form?
What should I do if I misplaced my provider's receipt showing Restylane® treatment?
- Please call or visit your provider to obtain a copy of the receipt. As a reminder, only an office receipt showing treatment with Restylane® is valid; a credit card receipt does not qualify.

How long will it take to receive my reward?
- Once your submission has been received and verified, it should take six to eight weeks to receive your reward.
Should I make copies of everything I submit to Restylane Rewards®?
- Yes, we strongly recommend you keep copies of everything you submit. In case there are questions, or your submission never makes it to the Restylane Rewards® facility, you have documentation to follow up.
What do I do if I receive a non-compliance notice (via e-mail or U.S. Postal Service) for my reward submission?
- The notice will indicate the reason your submission did not qualify for a reward. If you have the necessary information or materials indicated on the notice, you can resubmit to the Restylane Rewards® Center address listed above.
How can I check on the status of my reward?
- Please visit the Restylane Rewards® section of www.RestylaneUSA.com and click on My Restylane Rewards, or call toll-free 800-211-6794, 9 a.m. to 5 p.m. Eastern time.
Is there a deadline for sending in my submission?
- Each redemption form must be postmarked no later than 30 days after your Restylane® treatment date.
Who do I contact if I do not receive my reward within six to eight weeks from when I sent in my redemption form?

Does a treatment in more than one area qualify for Restylane Rewards®?
- Yes. However, there is a maximum of three treatments during your first 12-month period as a rewards member, with a 6-month maximum allowable time between treatments. When you achieve Premium Status, there is a 30-day minimum allowable time between treatments, Restylane® treatment in more than one area in the same office visit (e.g., nasolabial folds and glabellar creases) will only count as one treatment.
