To place your Restylane^® and Perlane^® order online, you will need to register with our ordering system. If you have previously registered in the Restylane Distribution Program, you must also register with our ordering system to create your online account, user name and password.

As part of the registration process, you will need to provide the following information:

• State License Number
• First Name
• Last Name
• ZIP Code

Your order can be charged to your established account or paid by credit card. After placing an order, you can return to the site to view your account and track your orders of Restylane or Perlane. We encourage you to visit the site often for the latest information.

Please make sure you enter your name and ZIP code as they appear on your last invoice for Restylane or Perlane. The information you provide must match our database exactly. If you are registered as more than one business entity in the Restylane Distribution Program in more than one state, you will need to establish a separate user name and password for each entity.
